To facilitate the flow of Emergency Department (ED), PACU, ICU patients to the appropriate inpatient/observation bed.
Roles & Responsibilities:
Responsible to plan, direct, implement and evaluate the flow of patients within the hospital from various points of entry.
Responsible to lead the Bed Management Huddle to review patients waiting for beds in the Emergency Department, PACU, ICUs Pending/Confirmed discharges; Reviews OR schedule and identify potential patients for conversion.
The Gate-keeper of beds within the system and, as such, has primary responsibility to facilitate patient throughput from various points of entry (Emergency Department, Peri-Operative Services, Surgical Admitting, Direct admits, transfers from other hospitals and procedural locations, such as the Cardiac Cath lab, Endoscopy) based on appropriate aggregation and level of care. The following departments share responsibility in the management of bed access - Transport, Environmental Services, Patient Care Services and Clinical Department.
Beds are assigned based on medical/surgical clusters/units. Another function is to assist in facilitating transfer of patients from hospitals with the assistance of the Transfer Center.
Review the EMSTAT and PRISM/EPIC system for appropriateness of bed assignment.
Monitors Bed Teletracking/ADT system to review patients who are ready to move, tracks the bed availability and patient flow constantly ensuring system is accurate and that units are entering confirmed discharges and staffed beds. This allows the department to prepare for overflow in a timely manner if needed.
Monitors Bed Teletracking/ADT system and escalate dirty beds with Environmental Services.
Reviews appropriateness of any blocked beds in the system.
Rounds on units to assess unit needs and facilitate with nurses “pulling” patient report.
If it is determined at any of the Bed Management Meetings, that the volume of patients exceeds the anticipated capacity criteria, the Director of the Emergency Department or designee, Director of Patient Access or designee, CMO, Director of Nursing (Supervisor) will activate the Surge Plan
BSN degree required
5 years of Medical Surgical or Emergency Department experience.
Previous Assistant Nurse Care Coordinator (ANCC) or Charge Nurse Role preferred
NYS RN license and registration required
