Primary Care provides acute and chronic care needs for Eastern Oklahoma VA Health Care System including a variety of services for adult and elderly Veterans. The PACT RN Care Manager uses nursing processes and evidence based practice to collaborate with the Team (Patient, Primary Care Provider, RN Care Manager, LPN, and MSA) and larger team (family/caregiver, internal and community based services involved in providing care) in developing the patient driven holistic plan for life. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
